We are committed to making the referral process as seamless as possible for you and your patients, we handle prior authorization and financial navigation for all referrals, provide clear out-of-pocket cost estimates, and help patients reduce costs through financial assistance and co-pay support. Plus, referrals are accepted from all physicians.
How to Refer
You may submit referrals through one of the following methods:
- Online Referral Form: Complete a secure online referral form and submit it directly through our website.
- Phone Referral: Call our referral team to provide patient details and initiate the referral process.
- Fax Referral: Download a referral form and fax your submission. For all referrals please include: demographics, insurance information, lab results, current medications, and recent visit notes.
- Epic Referral: Providers with access to Epic are able to submit referrals directly through Epic.
